What is the Difference Between a Plastic Surgeon And a Cosmetic Surgeon?

There is a significant and noticeable different between plastic surgeons and "cosmetic surgeons" that the general public does not fully understand. Plastic Surgeons are trained in cosmetic surgery as well as reconstructive surgery, this is a very specific kind of physician. "Cosmetic surgeon" is a non-specific term that can be a legitimate plastic surgeon or someone whom is untrained in plastic surgery who does these procedures (often poorly and dangerously). If a surgeon uses the term "cosmetic surgeon" or "aesthetic surgeon", they are likely not a plastic surgeon. They may be excellent dermatologists, OB/Gyn doctors, family practice doctors, internal medicine doctors or dentists to name a few but that does not make them qualified to do cosmetic surgery.

To become board certified by the American Board of Plastic Surgery, a plastic surgeon must first complete intensive training in an accredited training program. Thereafter, the surgeon must pass rigorous written and oral examinations given by the board. The American Board of Plastic Surgery is one of only 24 accredited specialty boards recognized by the American Board of Medical Specialties (ABMS) and is the only board recognized by the ABMS to certify a surgeon in plastic surgery. There are a number of other boarding organizations that include "facial", "aesthetic" or "cosmetic" in the title. These are not recognized by the ABMS and do not have the same training requirements. The American Board of Cosmetic Surgery (ABCS) is not recognized by the ABMS and requires no prerequisite training in plastic surgery. According to the ABCS website, this is superior to any other boards for cosmetic surgery as it is the only board certifying doctors in cosmetic surgery!!! I find this extremely offensive. In fact a "cosmetic surgeon" practicing in my local area whom is actually in primary care tells his patients that he is "better than a plastic surgeon because he is a cosmetic surgeon and fixes the plastic surgeons mistakes!!!"

Very few, if any plastic surgeons are certified by the American Board of Cosmetic Surgery. No non-plastic surgeons are certified by the American Board of Plastic Surgeons.

The new H+ method of hair transplantation

Until recently, most patients requiring treatment had to undergo invasive strip surgery (removing part of the scalp during an operation). As a result of the obvious physical limitations of this invasive procedure, many were allowed to go almost totally bald before they could be treated. A lot of these newer techniques have been perfected in Dublin. Rejuvenate Magazine spoke with Dr. Patrick Treacy about the option of the new H+ method of hair transplantation.

Over the past few years unit follicular hair transplant has emerged as the most reliable hair transplant procedure of hair transplantation surgery. As the name suggests, the graft in this case consists of a single follicular unit as it exists in nature. The follicular unit contains sebaceous glands as well as nerve and blood tissue. It is for this reason that it needs to be extracted as an intact unit and transplanted as an intact unit, to be able to grow into a hair. There are basically two types of technique.

1 Follicular Unit Transplantation (FUT) is a hair restoration procedure where hair is transplanted exclusively in its naturally occurring groups of 1-4 hairs. These groups, or follicular units, are obtained through the microscopic dissection of tissue taken from a single donor strip or extracted directly from the donor area.

You mentioned genes. How do they play a role in hair loss?

It was previously believed that baldness was inherited from the maternal grandfather. While there is some basis for this belief, it is now known that both parents contribute to their offspring's likelihood of hair loss. Large studies in 2005 and 2007 showed the importance of the maternal line in the inheritance of male pattern baldness. German researchers called the main 'balding' gene, 'ARG' or the androgen receptor gene. The presence or absence of a specific variant in this gene is related to baldness.

How common is this condition?

This is a good question. Studies show nearly all men have some baldness by the time they are in their 60s. However, the age the hair loss starts is variable. One large Australian study showed the prevalence of mid-frontal hair loss increases with age and affects about three in ten 30 year old males and half of 50 year old males.

With evolving techniques like the newer single hair follicle extraction technique, follicles are extracted one by one from the donor area (back of the head) and re-implanted with the unique patented devices. The H+ method is based on the use of genetic testing as a means of screening, the use of growth factors, 633nm light for fibroblast stimulation and the use of quality control during the hair transplant procedures. The hair transplant doctor needs only a single surgical assistant to help him during the follicle hair transplants. The procedure also ensures maximum care to the grafts. The hair transplant doctor can decide upon the exact number of grafts to be removed while he is removing them one-by-one.

Another advantage of the H- transplant procedure is that it allows the hair transplant surgeon to extract as many grafts as is actually needed and on average, we usually extract and place 3,000-5,000 hairs per day. Bandaging is not necessary following the procedure, allowing the patient to leave the clinic looking just as they did on entering. Unlike the 'strip' technique, there is no scarring with the H+ method and a patient can shave his head or wear all the current cuts and styles. In addition, there is also no waiting period between H+ procedures and hair from all over the body can be used.

When did hair transplantation begin?

Modern hair transplantation began in the 1950s based upon the pioneering work of New York dermatologist Dr. Norman Orentreich. According to medical myth, he was doing a study on vitiligo, transferring 4 mm punch skin grafts in an attempt to determine whether vitiligo was "donor" or "recipient" area dominant. The patient noted that a punch graft that had been taken from a hair-bearing area, grew hair, after it was placed into a non-hair-bearing area. Previously it had been thought that transplanted hair would thrive no more than the original hair at the "recipient" site.

Why did transplants by 'punching' stop?

Because the H+ technique may continue to place hair over a longer period we have decided to allow patients to join a 'Samson' club. Can you explain the 'minigrafts' and micrografts technique?

In the late 1980s, Limmer introduced the use of the stereo-microscope to dissect a single donor strip into small micrografts. Micrografts usually consist of one or two hairs per graft and 'minigrafts' contain 3-8 hairs per graft. These smaller grafts, when properly placed, provided a more natural, less abrupt appearing hairline in contrast to other methods. However, they could also contain up to 12 hairs producing a bulky tufted appearance leading to a 'doll's head', or 'toothbrush' effect.

What is the most popular technique?

During the past few years, follicular unit transplant grafts (FUT) have become popular due to a more natural effect. Follicular units were first described in the medical literature by Headington in 1984. Follicular Unit Transplantation has its roots in the single-strip harvesting method and microscopic graft dissection technique developed by Dr. Bobby Limmer in the late 1980s and published in 1994. In 1995, Bernstein and Rassman published the first paper on "Follicular Unit Transplantation," where hair is transplanted exclusively in naturally occurring groups of 1-4 hairs.

What do you mean by the 'strip' technique?

The strip method is really an invasive surgical procedure requiring the attendance of a plastic surgeon and general anaesthesia. During the procedure, a long strip of scalp, approximately 20- 25 cm in length and 1 cm to 2.5 cm in width of hair-bearing skin is removed from the donor area using a scalpel, cutting through nerves and major blood vessels of the scalp. The donor area is then sutured (stitched) closed and approximately 30 days are required for full healing. In order to avoid stretching of the donor scar, two layers of donor stitches must be used and left in the skin for at least two weeks.

What are the problems with the strip technique?

. Should a patient shave their hair short in the donor area, a line or scar will always be visible. A patient forever gives up the option to shave their head or have a short haircut after a strip procedure. Although there are many good proceduralists of this technique, I personally would consider 'strip surgery' an out-dated procedure.

What are the problems do you have with the strip technique?

The patient can develop scarring or neurological problems of sensation in the back of the head post procedure. Another disadvantage of strip extraction is that the patient has to wait nine months for the area to heal fully before considering or being able to undergo a second procedure

In performing a lower lid Blepharoplasty, I remove the “herniated fat” causing the puffy appearance of the lid, tighten the laxity of the lid margin itself with a lateral canthopexy, elevate and suspend the underlying obicularis oculi muscle and remove a conservative amount of skin.”

In addition to examining the eyelids, Dr. Parker will also evaluate the position of the patient’s eyebrows as well as any wrinkling they may have between the eyebrows or on the forehead during the patient’s initial consultation. He will also discuss options to rejuvenate these as well, including the injection of Botox or surgical solutions to these areas. Surgical solutions include a brow lift or modified brow lift procedure to be done simultaneously with eyelid surgery, based on the individual patient’s anatomy and desires.

Finally, in order to achieve natural-looking results with Blepharoplasty surgery, Dr. Parker stresses the importance of choosing a surgeon who will perform a thorough pre-operative analysis of the eyelids and the areas around them. The surgeon should then carefully discuss the various surgical and non-surgical options with the patient to work out a detailed treatment plan.

“To make sure everyone is on the same page, I personally find it helpful to supplement these consultations by having the patient spend time with our professional photographer and computer image to review ‘imaged’ pictures of what we believe the patient will look like after surgery, in addition to actual before and after pictures of prior Blepharoplasty patients”.

Clearly, when it comes to a person’s face, a patient should never feel uncomfortable asking for some of the technical information provided by Dr. Parker above -- as an involved and informed patient will ultimately ensure the best results with eyelid surgery.

Laser Lipolysis is a procedure that 'liquefies' fat cells using a laser inserted under the skin. Finally there is a way to grow your own eyelashes, naturally. A new drug called Latisse has been introduced to solve this age-old problem. Made by the Allergan company, this drug is clinically proven to grow longer and thicker lashes.

Lashes 25% longer,106% thicker and 18% darker were reported at the end of a 16 week study, using Latisse. Applied to the upper eyelid only, changes were noted at the 8 week mark. With these numbers Latisse sounds like a mascara advertisement!

The clinical trial consisted of 280 members. 1/2 used the drug for 16 weeks. They applied the drug to the upper eyelid only. Latisse is not recommended for use on the lower eyelids. Latisse was well tolerated by the participates.

Latisse will not be an over- the- counter medicine. Cosmetic use will not covered by medical insurance. Medical conditions such as hypotrichosis, would probably be covered by insurance. It will cost about $120.00 for a 30 day supply, with 60 single use applicators.

Side effects were noted during the study. 3.6% of the members experienced eye itching. Permanent darkening of the iris and eyelid are also possible side effects. Although no one in the study reported these effects. Green and Hazel eyes could be at risk for these complications.

Allergan is the company introduced Botox to the world. While developing a drug for glaucoma, called Lumigan, researchers found the group using the new drug had dramatic growth of their eyelashes. The active ingredient identified to cause eyelash growth is called Bimatoprost. Allergan then created Latisse to take advantage of this effect. Some those who used Lumigan experienced a darkening of their iris and eyelids. Because Latisse uses 5% of this active ingredient, applied to the eye lash and not in the eye, these side effects are not expected. Lumigan is placed into the eye.

The life cycle of a normal eye lash follicle is 4 to 5 months, so when you stop using the drug, gradual lash length will be reduced, as lashes fall out and they are replaced by normal lashes.

Should breast tissue be screened for cancer after cosmetic surgery?

Analysis: Tissue screening after breast reduction, BMJ online

Young women undergoing cosmetic breast reduction surgery are being screened for cancer without their informed consent, according to a paper published on bmj.com today.

Breast reduction surgery (mammoplasty) is one of the most common procedures performed by plastic surgeons all around the world. For decades it has been common practice to test the removed tissue for cancer.

The incidence of cancer found after surgery is small and often clinically insignificant. But, if found, it can lead to further surgery of unproven benefit. So should the current practice of routinely testing tissue after surgery be abandoned, or should doctors discuss this issue in advance with the patient and ensure that they are aware of the possible consequences?

A team of breast surgeons based at the Royal Free Hampstead NHS Trust and Royal Free and University College Medical School in London describe finding cancer after a routine cosmetic operation on a 37 year old woman. The discovery led to a succession of further operations, but the team question the ethics of acting on test results when there is no evidence for benefit.

They also point out that it is often not possible to identify exactly where in the breast the tissue came from because tissue specimens are not orientated during surgery.

In an accompanying commentary, Tom Treasure, a Professor of Cardiothoracic Surgery at University College London suggests that the question of what is the best management of a patient with these findings in the future remains unanswered. "Not putting the tissue under the microscope may seem unacceptable," he writes, "but so is continuing surgical practices that may result in harm, without having evidence of benefit."

In a second commentary, ethicist Jeremy Sugarman suggests that even though the likelihood of the specimens being malignant is small, discussing this issue in advance should help to prepare patients to receive the news and to face the complex decisions that follow. He believes that this matter warrants careful, expert review. In the meantime, he suggests that obtaining informed consent for screening and orientating these specimens should help to mitigate some of the difficult ethical issues that are encountered in practice.

Like any potential patient, I welcome any procedures made to safeguard my health and would appreciate being informed of every aspect (and associated risk) of an operation. I would not appreciate being left in the dark, writes Tessa Boase, in a final commentary. She believes that the current practice of screening of breast tissue after reduction mammoplasty should be maintained and perhaps refined, but the patient should, from start to finish, be kept in the loop. "Who else, after all, is this screening supposed to benefit?" she says.

Cosmetic Surgery to alter the labia is the third most sought-after operation after facial and breast augmentation surgeries, the authors, psychologist Ada Borkenhagen and gynaecologist Heribert Kentenich, said.
As is the case in breast augmentation, the desired look is youthful. More precisely, the vulva should be completely covered by the labia. Thus, the most common cosmetic surgery among women seeking genital enhancement is the reduction of the labia — the folds of female external genitalia.

The surgery is meant to improve self-confidence and sex. It is even supposed to improve quality of life in general. But these claims are inadequately supported by studies, according to Borkenhagen und Kentenich. Complications cannot be ruled out.

And for men who think size does matter, the Andro-Penis, which extends penis size by a third, would come as a boon.

Experts from San Giovanni Battista Hospital at the University of Turin, Italy, suggest that the treatment could provide a viable alternative to surgery.

The independent clinical study published in the March issue of BJU International found that males who wore a penile extender every day for six months were able to increase the flaccid length of their penis by up to 32% and their erectile function by up to 36%.

The device comprises a plastic ring, two dynamic rods that produce the traction and a silicon band to hold the penis in place.

I don’t know why this is, but we are more critical of ourselves and other women than men are. Either they are more acceptable to our little (or big) physical handicaps, as long as they are getting some attention or they just shut up because if they complain, they know they aren’t getting any attention. LipoSelection and SmartLipo

LipoSelection

LipoSelection is an ultrasonic liposuction procedure, meaning it uses ultrasonic energy as a means of fat removal. The ultrasound technology breaks up and softens the fat that is to be removed. Unlike other fat removal methods, LipoSelection leaves the surrounding nerves, blood vessels, and tissue virtually unharmed. After a local anesthetic is applied, the surgeon will fill the area to be treated with a saline wetting solution to numb the site and shrink blood vessels. This helps minimize post-surgical bruising and swelling. A small probe emitting high frequency sound energy is used to break up fat on contact with minimal damage to surrounding tissues. Fat may then be easily extracted with liposuction tubes.

LipoSelection allows physicians to sculpt and shape delicate areas of the body, such as the arms, chin, neck, and inner thighs. It can remove fat from any area treatable with traditional liposuction techniques: abdomen, thighs, buttocks, knees, hips, back, arms, male breasts, love handles, chin, and neck areas.

SmartLipo is different in that it uses a carefully standardized laser to melt fat deposits through the top layers of the patient's skin. The laser actually shatters fat cells, and the resulting oily, liquid substance is then removed through a tiny incision in the skin by the surgeon. The small laser can also seal blood vessels as it zaps fat, so there is less swelling, bleeding and bruising than with traditional liposuction. Research has shown that little, if any, fat is reabsorbed back into the body. The minimal invasiveness of the procedure allows patients to see faster results and to experience fewer complications.

In addition, SmartLipo’s patients need only a local anesthetic for this procedure, whereas liposuction requires general anesthesia. Because the patient is not completely unconscious, SmartLipo carries much fewer risks than traditional liposuction, and the recovery time is much shorter.

Almost anyone is a candidate for SmartLipo. In general, the ideal candidate is in good health and within 25lbs of their ideal weight, has one or more areas of body fat deposits that do not respond to diet and exercise and would like to get rid of them permanently.

A few of our patients even return to work immediately, almost like a "lunchtime Liposuction". Of course we normally advise a day or two of rest. During the procedure, there is no pain. After the anesthesia has worn off, it only feels like the aches and pains that follow a vigorous workout. This is unlike in traditional liposuction where a patient feels as if a truck was driven over the body. The bruises too are much less with Smartlipo.

The fiber-optic cannula is inserted through a small puncture in the skin usually along creases that conceal the scar. As the laser melts the fats, it also coagulates the loose ends of small blood vessels. Compression garments are optional as the area will not bleed very much after the laser is removed. The granulation tissue (healing-tissues) does the rest of the work in contracting the treated area as well as the overlying skin.

Almost "normal" skin after 3 months It would be very hard to find the insertion points or tell by looking at the skin that any liposuction
has ever been done. Skin contracts with the healing-tissues that replace the melted fats. Smartlipo is safer than traditional liposuction because there is less bleeding and anesthetic drugs used. In fact, in doing the procedure under tumescent technique, we have eliminated a lot of risks normally associated with general anesthesia.
